Abstract

Актуальність. Для ефективного лікування захворювань очей вірусної етіології необхідний комплексний підхід, який включає загальне і місцеве застосування противірусних та імуномоделюючих препаратів, патогенетичних і симптоматичних засобів. Біопрепарати на основі живих мікробних культур застосовуються в медицині та ветеринарії для корекції мікрофлори шлунково-кишкового тракту, для боротьби з вірусними і бактеріальними інфекціями. Важливою роллю пробіотиків є їхня здатність підвищувати специфічну і неспецифічну імунну реактивність організму хазяїна, посилювати клітинну і гуморальну відповідь. При введенні пробіотиків активізується продукція цитокінів, особливо інтерферону. Мета дослідження. Підвищення ефективності лікування аденовірусних кон'юнктивітів з включенням в комплексну терапію індуктора ендогенного інтерферону субаліну. Матеріали та методи досліджень. У дослідження включено 73 хворих (146 очей) з аденовірусними кон'юнктивітами у віці від 19 до 72 років, середній вік (М +- м) – 38,2 + - 1,1 років. Всі хворі з аденовірусними кон'юнктивітами були поділені на 2 клінічні групи: основна (43 хворих, 86 очей), контрольна (40 хворих, 80 очей). Групи були порівнянні і рандомізовані за статтю, віком, клінічними проявами захворювання. Пацієнти основної групи отримували базисне лікування і додатково пробіотик субалін: по 2 краплі кожні 2 години до повного одужання (1 ампулу препарату розчиняли в 5 мл фіз. розчину). Хворі контрольної групи отримували лише базисне лікування. Базисна терапія хворих з аденовірусним кон'юнктивітом, незалежно від групи до якої вони відносилися, включала застосування противірусних, антисептичних, протизапальних, десенсибілізуючих лікарських засобів. Результати та їх обговорення. Застосування пробіотика субаліну додатково до традиційної терапії в основній групі хворих сприяло позитивній динаміці з боку органу зору. Так, зникнення набряку повік і кон'юнктиви в основній групі хворих з аденовірусним кон'юнктивітом відбувалося на 2,39 дні раніше в порівнянні з контрольною групою хворих з цією патологією (p <0,05). В основній групі хворих одужання відбувалося швидше на 6,01 дня (p <0,01). У процесі лікування у всіх хворих послідовно підвищувалася корегована гострота зору. Застосування пробіотика у всіх клінічних групах сприяло більш високій гостроті зору у хворих в основній групі після проведеного лікування: приріст гостроти зору більше 0,7 в основній групі хворих з аденовірусним кон'юктивітом склав 23%, в контрольній - 9, 2%. Висновки. Використання субаліну в основній групі у складі комплексної терапії сприяло якнайшвидшому зникненню фолікулів та гіперемії кон'юнктиви, скороченню тривалості лікування. Summary. Inflammatory diseases of the conjunctiva occupy one of the first places among the pathologies of the visual organ. Adenovirus infection makes significant contribution to the development of inflammation in the conjunсtival cavity. Insufficiently active and effective treatment of acute virus, including the adenovirus lesions of the eye, results in a transition into chronic form. This form is characterized by slow and sluggish duration, short periods of remission and frequent relapses, and brings discomfort to the patient for a long time. Nowadays, there is an ongoing search for new drugs in ophthalmic practice that do not focus on chemotherapy and have antiviral, antibacterial affects and possess factor of specific immune resistance of the human body. Bio-drugs, based on alive microbial cultures are usually used in human and veterinary medicine to correct microflora of the gastrointestinal tract, to fight viral and bacterial infections. An important role of probiotics is their ability to increase the specific and non-specific immune reactivity of the host organism, and enhance it's cellular and humoral response. When administered, probiotics activate production of cytokines, particularly interferon. Purpose of the research. Improvement of the treatment efficiency of adenoviral conjunctivitis with inclusion of endogenous interferon subalin inducer in complex therapy. Materials and methods. Examination and treatment was performed in 73 patients (146 eyes) with adenoviral conjunctivitis. They were divided into 2 clinical groups: main group of 43 patients (86 eyes) and control group of 40 patients (80 eyes) group. Patients of both groups received traditional therapy which includes antiviral, antiseptic and anti-inflammatory drugs. Patients of main group were additionally prescribed with probiotic subalin: 2 drops every 2 hours. Results and their analysis. The use of subalin probiotic in addition to conventional therapy in the main group of patients helped by the positive dynamics of the visual organ. Thereby, the disappearance of the eyelids and conjunctiva edema, in patients with adenoviral conjunctivitis happened 2.39 days earlier compared to the control group of patients with the same disease (р < 0,05). The main group of patients recovery was faster by 6.01 days (p <0.01). During treatment corrected visual acuity consistently increased in all patients. The use of probiotics in all clinical subgroups of patients led to higher visual acuity in the main group of patients after performed treatment: increase in visual acuity more than 0.7 in the main group of patients with adenoviral conjunctivitis was 23% , and in the control group - 9.2%. Conclusion. Usage of subalin in the main group in the complex therapy, undoubtedly contributed to faster resortion of conjunctival hemorrhage, disappearance of follicles and conjunctival hyperemia. The treatment duration is shorter if to compare with treatment duration of control group.
