Abstract

Мета роботи – вивчення стану мікроциркуляторного русла у хворих на оніхомікоз. Матеріали та методи. Проведено обстеження 72 хворих на оніхомікоз (47 чоловіків і 25 жінок у віці від 25 до 77 років; 62 % хворих були старіше за 50 років) з давністю грибкового ураження нігтьових пластинок від 9 місяців до 23 років. Усім хворим до початку і після лікування проводився загальний і біохімічний аналіз крові (загальний білок, АЛТ, АСТ, лужна фосфатаза, білірубін, сечовина, холестерин, ліпопротеїди, глюкоза, креатинін, тимолова проба, С-реактивний білок), мікроскопічне і культуральні дослідження лусочок і ураженої частини нігтьових пластинок. Для вивчення стану мікроциркуляторного русла у хворих на оніхомікоз застосовано біомікроскопію судин бульбарної кон’юнктиви. Результати та обговорення. У хворих на оніхомікоз старших вікових груп, які були під нашим спостереженням, у всіх відділах мікроциркуляторного русла бульбарної кон’юнктиви виявлено порушення мікрогемодинаміки у вигляді підсилення нерівномірності калібру судин та наявності їх звивистості, особливо венул, меандричної звивистості, наявності мікроаневризм і венулярних сакуляцій, утворення судинних клубочків, спазму артеріол і дилатації венул зі зниженням у результаті цього артеріоло-венулярного коефіцієнту до 1: 4 – 1: 6 та менше. У хворих молодшої вікової групи порушення мікрогемодинаміки мікроциркуляторного русла носили менш виражений характер. Висновки. При плануванні комплексного методу лікування хворих на оніхомікоз необхідно індивідуальний підхід з урахуванням поширеності і давності процесу, наявності супутніх захворювань, стану імунної системи та мікроциркуляторного русла. Purpose – to study the microvasculature status in patients with onychomycosis. Materials and methods. 72 patients with onychomycosis (47 men and 25 women aged 25 to 77 years; 62 % of them were older than 50 years) having the duration of the nail plate fungal infections from 9 months to 23 years were examined. All patients before and after the treatment were subjected to the general and biochemical analysis of blood (total protein, ALT, AST, alkaline phosphatase, bilirubin, urea, cholesterol, lipoproteins, glucose, creatinine, thymol test, C-reactive protein), microscopic and cultures studies of scales and the affected part of the nail plate. The vascular biomicroscopy of bulbar conjunctiva was applied to study the state of the microvasculature in the patients with onychomycosis. Results and discussion. The patients with onychomycosis of the older age groups being under our supervision revealed the infringements of microhemodynamics in all departments of the microvasculature bulbar conjunctiva in the form of increase in the non-uniformity of vascular caliber and presence of vascular tortuosity, especially of venules, meander tortuosity, presence of microaneurysms and venular sacculation, the formation of vascular glomeruli, arterioles’ spasm and venules’ dilation resulting in the reduction of the arteriolo-venular ratio up to 1:4 - 1:6 or less. In the patients of the younger age group the infringements of microhemodynamics displayed less marked character. Resume. When planning the complex treatment of patients with onychomycosis, an individual approach is required taking into account the extent and duration of the process, presence of concomitant diseases, status of the immune system and the microvasculature bed.
