Abstract

Целью работы было установить гемодинамические (по данным суточного мониторирования артериального давления (СМАД)) детерминанты прогрессирования ГЛЖ у больных гипертонической болезнью (ГБ) ІІІ стадии, перенесших инфаркт миокарда и нарушение мозгового кровообращения (НМК). Полученные результаты свидетельствуют о том, что больные ГБ ІІІ стадии с постинфарктным кардиосклерозом характеризовались максимально высокой степенью ГЛЖ за счет увеличения внутримиокардиальных размеров левого желудочка, а у больных, перенесших НМК, при меньшей степени ГЛЖ наблюдалось снижение ударного объема крови и повышение общего периферического сосудистого сопротивления. У больных ГБ ІІІ стадии и постинфарктным кардиосклерозом ведущие позиции в прогрессировании ГЛЖ занимали ночная вариабельность среднего гемодинамического и систолического АД, максимальный уровень систолического и пульсового АД ночью и их минимальные дневные значения. У больных ГБ ІІІ стадии, перенесших НМК, наиболее выраженное влияние на ГЛЖ оказывали минимальный уровень пульсового АД в утренние часы и величина минимального систолического АД ночью. Выявленные особенности ассоциаций между показателями СМАД и величиной индекса массы миокарда левого желудочка у больных ГБ с постинфарктным кардиосклерозом и перенесенным НМК необходимо учитывать при назначении антигипертензивной терапии с целью максимальной оптимизации вторичной профилактики. Rationale. Left ventricular hypertrophy (LVH) is recognized as an independent risk factor for cardiovascular morbidity and mortality. Therefore the search for the causes of LVH and analysis of factors affecting the myocardial mass and changes in left ventricular geometry remains an important issue. The aim of this investigation was to establish a hemodynamic (according to ambulatory blood pressure monitoring (ABPM)) features leading to progression of LVH at patients with the third stage of arterial hypertension after myocardial infarction (MI) and stroke. Methods. The study included 46 patients (36 male and 10 female) with hypertension stage III after myocardial infarction, and 26 patients (16 male and 10 female) after stroke. The average age of observed patients at group after MI was 57.87 ± 1.17 and 54.08 ± 1.76 years at patients after stroke, disease duration – 10.46 ± 1.11 and 12.12 ± 1.83 years respectively. The main parameters in myocardial structure and cardiac output were evaluated by echocardiography. Monitoring ABPM was performed using a portable non-invasive system “ABPM-04” (MEDITEX, Hungary). Results. It was shown that at patients with AH stage III after myocardial infarction was highest degree of LVH by increasing of the intramyocardial size of the left ventricle and at patients after stroke was observed lower degree of left ventricular hypertrophy with reduction of stroke volume and increasing of total peripheral vascular resistance. Comparative analysis of ABPM in observed patients depending on the previously suffering complications showed that the amount of the systolic, diastolic and pulse pressure rising in the daytime and at night in patients after stroke compare with other patients was more significant (p < 0.0001), especially at night. The leading position at LVH progression in patients with stage III AH after MI took night variability of the average hemodynamic and systolic blood pressure, the maximum level of systolic blood pressure and pulse during the night and their minimum daily values. In patients with AH after stroke the most pronounced effect on left ventricular hypertrophy have a minimal level of pulse pressure in the morning and the minimum value of systolic blood pressure at night. Conclusion. The revealed features of associations between indicators of the ABPM value and LVH in hypertensive patients after myocardial infarction, and stroke must be considered at administration of antihypertensive therapy for maximum optimization of secondary prevention.
