Abstract

Клініко-лабораторно обстежено 80 хворих на інфекційні ураження великих шкірних складок (ІУВШС); у 47 (58,75 %) з них виявлено мікст-інфекції – асоціації дерматоміцетів (Trichophyton spp., Epidermophyton spp.) і дріжджоподібних грибів (Candida spp.) з бактеріальними збудниками. Моноінфекції було виявлено у 33 (41,25 %) хворих. При проведені кластерного аналізу всіх отриманих даних ми розподілили хворих у дві клініко-терапевтичні групи, у першу чергу, базуючись на особливостях мікробного пейзажу великих шкірних складок. Це дало можливість диференційовано проводити терапію пацієнтам різних груп. Хворим першої клініко-терапевтичної групи, де збудниками ІУВШС були моноінфекції, призначались етіологічні препарати (Ламікон, Спорагал, еритроміцин), гепатопротектори (Дарсіл), імуномодулятори (Субалін, Тіотриазолін), адаптогени (Гліцисед); зовнішня терапія (Ламікон, Клотримазол, Фузідерм), а хворим другої клініко-терапевтичної групи (з мікст-інфекціями) – ітраконазол (Спорагал), Цинарікс, Ліастен; зовнішньо – крем Триакутан. Отримані сприятливі клініко-лабораторні результати лікування з нормалізацією показників імунного статусу, особливо IL-10 та IL-12, а також біохімічних аналізів. 80 patients with infectious lesions of the large skin folds (ILLSF) were examined clinically and laboratory; 47 (58,75 %) of them revealed mixed infection – the association of dermatomycetes (Trichophyton spp., Epidermophyton spp.) and yeasts (Candida spp.) with bacterial pathogens. Monoinfections were detected in 33 (41.25 %) patients. When conducting the cluster analysis of all the data, we distributed the patients among two clinical and therapeutic groups, primarily based on the features of the microbial landscape of the large skin folds. This gave a chance to carry out the differentiated treatment of the different groups of patients. The patients of the first clinical-therapeutic group (with monoinfection of ILLSF) took the etiological medications (Lamikon, Sporagal, erythromycin), hepatoprotectors (Darsyl), immunomodulators (Subalin, Thiotriazoline), adaptogens (Glycised); the external therapy (Lamikon, clotrimazole, Fuziderm), and the patients of the second clinical-therapeutic group (with mixed infection) – itraconazole (Sporagal), Cynarix, Liasten; externally – cream Triacutan. Favorable clinical and laboratory results of the administered treatment together with normalization of the immune status, especially of IL-10 and IL-12 and biochemical analyzes were obtained.
