Abstract

Когнітивна дисфункція залишається важливою проблемою в соціальній адаптації пацієнтів, які перенесли тяжку термічну травму. Мета роботи: Виявити залежність між рівнем ендогенної інтоксикації та розвитком когнітивної дисфункції у хворих з тяжкою термічною травмою. Матеріали і методи: Показники когнітивної дисфункції та рівня ендогенної інтоксикації визначалися у двох групах пацієнтів, які отримали тяжку термічну травму: 1 група (n = 14) – індекс тяжкості ушкодження (ІТУ) 78,5 ± 11,3 од.; 2 група (n = 21) – ІТУ 159,4 ± 40,9 од. Група контролю (n = 28) включала пацієнтів з термічною травмою до 10 % поверхні тіла. У всіх пацієнтів визначалися показники пам’яті та мислення, проводилась оцінка психоемоційного статусу на підставі психологічних тестів. Як показники рівня інтоксикації визначалися лейкоцитарний індекс інтоксикації модифікований (ЛІІм) і рівень загального білка крові. Результати: У групах дослідження рівень когнітивних функцій був нижче показників групи контролю і не відновлювався на момент виписки. Рівень ЛІІм також перевищував показники контрольної групи; максимальні дані в групах дослідження спостерігалися в різні періоди опікової хвороби (ОХ) у залежності від тяжкості стану. Також спостерігалися відмінності в результатах тесту самопочуття, активності і настрою (САН) у пацієнтів 1 і 2 груп. Була виявлена кореляція між показниками ендогенної інтоксикації та ступенем когнітивної дисфункції. Висновки: Ступінь когнітивної дисфункції у пацієнтів з тяжкою термічною травмою залежить від рівня ендогенної інтоксикації; дана залежність більш виражена у пацієнтів з вкрай тяжким перебігом ОХ. Cognitive dysfunction is one of the major problems in the social adaptation of patients after severe thermal injury. Purpose: to determine the relationships between the degree of endogenous intoxication and the development of cognitive dysfunction in patients with severe thermal injury. Materials and methods: The indices of cognitive dysfunction and the degree of endogenous intoxication were determined in two groups of patients experienced severe thermal injury: 1st group (n = 14) – the injury se– the injury se the injury severity index (ISI) 78.5 ± 11.3 units; 2nd group (n = 21) – ISI 159.4 ± 40.9 units. The control group (n = 28) included patients with thermal injury less than 10 % of the body surface. The cognitive dysfunction: memory status, ideation and psycho-emotional status (test-questionnaire concerning well-being, activity and mood (WAM)) was studied in all the patients. The degree of endogenous intoxication was determined by the leukocytic intoxication index modified (LIIm) and the total protein level. Results: In the study groups the degree of cognitive functions was lower than in the control group and was not restored till the discharge time. The LIIm level was also above the control group one; the maximum rates in the study groups were observed in the different periods of burn disease (BD) depending on the severity of the state. The differences were observed in the results of WAMtest in patients of the 1st and 2nd groups. There were correlations between the degree of endogenous intoxication and the cognitive dysfunction. Conclusions: The severity of cognitive dysfunction in the patients with severe thermal injury depends on the level of endogenous intoxication; this dependence is more pronounced in patients with extremely severe BD course.
