Abstract
Дисертаційна робота присвячена комплексному вивченню проблеми післяекстракційних ускладнень, переважно запального характеру (біль, набряк, контрактура), у пацієнтів з ретенцією та дистопією третіх молярів нижньої щелепи. Актуальність дослідження обумовлена значною поширеністю даної патології у соціально активного населення, яка за статистичними даними спостерігається у 54,9 % випадків і призводить до порушення нормальної життєдіяльності, часткової непрацездатності та збільшення строків одужання. Загальна практика, коли більшість хворих із утрудненим прорізуванням третіх молярів нижньої щелепи лікується в умовах стаціонару, що становить 22-28 % від кількості госпіталізацій, є згубною для системи охорони здоров’я країни, що стало особливо відчутним під час перевантаження закладів на тлі пандемії COVID-19. Залучення амбулаторної ланки до активного розвитку та формування сучасного вектору хірургічної стоматології «wisdom surgery» повинно знизити навантаження на стаціонари та забезпечити потребу населення у доступній спеціалізованій допомозі. На сьогоднішній день досягнуті значні успіхи в профілактиці та лікуванні названих ускладнень за рахунок впровадження новітніх методів у медичну практику. Однак деякі питання стосовно раннього попередження та усунення запальних ускладнень в ділянці хірургічного втручання не достатньо вирішені і потребують поглибленого вивчення особливостей остеогенезу та пошук шляхів удосконалення способів регенерації кісткової тканини, особливо у пацієнтів зі значною її втратою. Мета дослідження – підвищення ефективності хірургічного лікування пацієнтів з ретенцією третіх молярів на нижній щелепі шляхом оптимізації процесів остеогенезу та попередження післяопераційних ускладнень в зоні післяекстракційних кісткових дефектів. У роботі використовували сучасні клінічні, біохімічні, мікробіологічні та статистичні методи дослідження. Для визначення структури та частоти виникнення запальних ускладнень, пов'язаних з ретенцією третіх молярів нижньої щелепи, нами був зроблений ретроспективний аналіз 814 історій хвороб пацієнтів працездатного віку (від 19 до 60 років, середній вік склав 39,5±4,6 роки), які знаходились на стаціонарному етапі лікування у щелепно-лицевому відділенні КЗ «Дніпропетровська обласна клінічна лікарня ім. І.І. Мечникова» в період 2012-2015 рр. The relevance of the study is due to the high prevalence of this pathology in the socially active population, which, according to statistics, is observed in 54.9 % of cases and leads to disruption of normal functioning, partial disability and increased recovery time. The general practice, when most patients with difficult third molars of the mandible eruption are treated in the hospital, which is 22 – 28 % of the number of hospitalizations is detrimental to the health care system of the country, which was especially noticeable during the overload of institutions against the pandemic COVID-19. links to the active development and formation of a modern vector of surgical dentistry "wisdom surgery" should reduce the burden on hospitals and meet the needs of the population in affordable specialized care. To date, significant progress has been made in the prevention and treatment of these complications through the introduction of new methods in medical practice. However, some issues regarding the early prevention and elimination of inflammatory complications in the field of surgery are not sufficiently resolved and require in-depth study of osteogenesis and finding ways to improve ways to regenerate bone tissue, especially in patients with significant loss. The aim of the study is to increase the effectiveness of surgical treatment of patients with retention of third molars on the mandible by optimizing the processes of osteogenesis and eliminating postoperative complications in the area of postoperative bone defects. Modern clinical, laboratory and microbiological researches were used in the work. Statistical methods - to analyze the results of the study and evaluate the success of treatment. To determine the structure and frequency of inflammatory complications associated with retention of the third molars of the mandible, we performed a retrospective analysis of 814 case histories of patients with this diagnosis who were in the inpatient phase of treatment among people of working age (19 to 60 years, mean age 39.5 ± 4.6 years) in the maxillofacial department according toKZ "Dnepropetrovsk Regional Clinical Hospital named after I.I Mechnikov" in the period 2012-2015.
