Abstract

В статті розглядаються питанні оптимізації надання психіатричної допомоги населенню України в умовах обмежених ресурсів внаслідок як пандемії COVID-19, так і воєнного стану. Мета: запропонувати певні організаційні кроки, які дозволять ефективніше використовувати наявні обмежені ресурси в наданні психіатричної допомоги населенню країни. Запропоновано 12 кроків організаційного характеру, які не потребують суттєвого додаткового фінансування, проте можуть покращити ефективність використання обмежених ресурсів. Наведені рекомендації можуть бути імплементовані наказами Міністерства охорони здоров’я, наказами по лікарні, розповсюджені через курси тематичного вдосконалення або фахові школи на кафедрах післядипломної освіти закладів вищої медичної освіти. В основі змін пропонується взяти напрацювання доказової медицини та економічну доцільність. Пропонується використовувати препарати (в рамках державних програм) та методи обстеження лише з доведеною високою ефективністю обмежувати госпіталізацію до психіатричних стаціонарів лише невідкладними показаннями або необхідністю проведення стаціонарної експертизи. Також можливо відмовитися від рутинних методів дослідження, організуючи їх проведення на первинному рівні надання допомоги. Рекомендовані заходи мають наукове обґрунтування, але носять попередній характер та вимагають фахової дискусії усіх зацікавлених сторін: організаторів охорони психічного здоров’я, науковців, лікарів-психіатрів та представників пацієнтських спільнот та організацій. The article examines issues of optimizing the provision of psychiatric care to the population of Ukraine under the conditions of limited resources as a result of both the COVID-19 pandemic and military actions. The purpose : to offer specific organizational steps that will allow more effective use of limited available resources in providing psychiatric care to the country's population. 12 steps of an organizational nature are proposed, which do not require significant additional funding but can improve the efficiency of using limited resources. The above recommendations can be implemented by orders of the Ministry of Health, orders of the hospital, distributed through thematic improvement courses or professional schools at the departments of postgraduate education of institutions of higher medical education. It is proposed to take the development of evidence-based medicine and economic expediency as the basis of the changes. It is proposed to use drugs (within the framework of state programs) and examination methods with proven high efficiency, to limit hospitalization to psychiatric hospitals for urgent indications or the need for an inpatient examination. It is also possible to abandon routine research methods by organizing them at the primary level of care. The recommended measures have a scientific basis, but are preliminary and require professional discussion of all interested parties: mental health care organizers, scientists, psychiatrists, and representatives of patient communities and organizations.
