Abstract
Titovska S.O. The use of matrix metalloproteinase inhibitors in the complex treatment of patients with generalized periodontitis. – Qualifying scientific work on the manuscript. Thesis for the degree of Doctor of Philosophy in the field 22 – Medical Care, in specialty 221 – Stomatology. – Dnipro State Medical University, Dnipro, 2022. Generalized periodontitis is one of the most widespread human stomatological diseases; its severe forms are diagnosed in 11.0 % of the world's population. Despite the recognition of the microbial factor as leading in etiopathogenesis of periodontitis, antimicrobial treatment of gingivitis, added with removal of dental plaque, does not prevent the further development of inflammatory-destructive process in periodontal tissues. At the current stage of the periodontology, it is interesting to study the mechanisms of periodontal tissue damage initiated by a microbial factor, which include direct cytopathic effects, in particular apoptosis and pyroptosis of periodontal epithelial and connective tissue cells. At the same time, dysbacteriosis causes inflammation which leads to an increase of proteolysis products. They are a nutrient medium for periodontal pathogenic microflora which contributes to its growth. So, the study of the imbalance between the activity of proteolysis ferments matrix metalloproteinases and indicators of their regulation in oral fluid in patients with generalized periodontitis is necessary for the creation of effective methods of early diagnosis and preventive treatment of the disease, which includes the use of anti-inflammatory drugs, such as matrix metalloproteinase inhibitors. So, the aim of the presented research is to improve the effectiveness of complex treatment of generalized periodontitis by using matrix metalloproteinase inhibitors. Дисертація на здобуття наукового ступеня доктора філософії у галузі знань 22 – Охорона здоров’я, зі спеціальності 221 - Стоматологія. – Дніпровський державний медичний університет, Дніпро, 2022. Генералізований пародонтит – одне з найпоширеніших стоматологічних захворювань людини, складні форми якого діагностуються в 11,0 % населення світу. Незважаючи на визнання мікробного чинника провідним в етіопатогенезі пародонтиту, антимікробна терапія гінгівіту, доповнена видаленням зубних відкладень, не запобігає подальшому розвитку запально-деструктивного процесу в навколозубних тканинах. Звідси, на сучасному етапі розвитку стоматології інтерес представляє вивчення механізмів пошкодження тканин пародонта, ініційованих мікробним фактором, до яких належать прямі цитопатичні ефекти, зокрема апоптоз та піроптоз епітеліальних та сполучнотканинних клітин пародонта. Одночасно, дисбактеріоз викликає запалення, яке призводить до збільшення продуктів протеолізу. Вони є живильним середовищем для пародонтопатогенної мікрофлори та сприяють її росту. Таким чином, дослідження дисбалансу між активністю протеолітичних ферментів матриксних металопротеїназ та показниками їх регуляції в ротовій рідині в хворих на генералізований пародонтит є необхідним для створення ефективних методів ранньої діагностики та превентивного лікування захворювання, що включає застосування протизапальних препаратів, до яких належать інгібітори матриксних металопротеїназ. Отже, мета представленого дисертаційного дослідження – підвищення ефективності комплексного лікування генералізованого пародонтиту шляхом застосування інгібіторів матриксних металопротеїназ.
