Abstract
В сучасну епоху відбуваються зміни клінічного перебігу туберкульозу сечової системи, в тому числі туберкульозу нирок і сечовивідних шляхів. Також під впливом антибіотиків визначаються особливості специфічного туберкульозного запалення під час патоморфологічного дослідження біоптатів. Стійкі зміни симптоматики обумовлені природньою еволюцією МБТ, але останнім часом значно збільшується кількість мультирезистентних штамів. Природній патоморфоз також є наслідком зміни імунореактивності популяції, збільшення числа пацієнтів з ВІЧ-інфекцією. Індукований патоморфоз туберкульозу сечової системи під впливом широкого та часто необгрунтованого використання антибіотиків з бактеріостатичним впливом на МБТ не детермінований, але з часом призводить до зміни геному МБТ з мультирезистентністю. Ці зміни разом з іншими факторами впливають на клінічну симптоматику зі збільшенням кількості атипових форм захворювання. Під впливом антибіотиків відбуваються зміни у вогнищах типового специфічного туберкульозного запалення: починаючи з периферії з’являються ознаки неспецифічного запалення, а в центрі зникає казеозний некроз та клітини Пирогова–Лангерганса, більш вираженим стає фіброз, що зумовлює складнощі патоморфологічного підтвердження діагнозу. МБТ після пошкодження уроепітелію внаслідок призначення антибіотиків переходять в L-форми, які персистують в уражених органах, а неспецифічна флора адгезується на пошкодженому МБТ уроепітелії та утворює мікробні плівки з нечутливістю до антибактеріальних препаратів. In the modern era, there is a change in the clinical course of tuberculosis of the urinary system, including tuberculosis of the kidneys and urinary tract. Also, under the influence of antibiotics, specific features of specific tuberculous inflammation are determined during pathological examination of biopsy specimens. The persistent change in symptoms is due to the natural evolution of the M. tuberculosis, but the number of M. tuberculosis multi-drug resistant strains significantly increases in dynamics. Natural pathomorphosis is also a consequence of changes in the immunoreactivity of the population, an increase in the number of patients with HIV-infection. Induced pathomorphosis of tuberculosis of the urinary system under the influence of a wide, often unreasonable use of antibiotics with a bacteriostatic effect on the M. tuberculosis is not deterministic, but over time leads to permanent changes in the M. tuberculosis genome with MDR. These changes, along with other factors, influence the clinical symptoms with an increase in the number of patients with erased, atypical forms of the disease. Under the influence of antibiotics, changes occur in the foci of typical specific tuberculous inflammation, starting from the periphery of the foci, signs of nonspecific inflammation appear, and in the center caseous necrosis and Pirogov–Langans cells disappear, and foci are more pronounced fibrosis, which creates difficulties in pathomorphic confirmation of the diagnosis. After the damage to the uroepithelium, the M. tuberculosis becomes L-shaped, persisting in the affected organs, and the nonspecific flora adheres to the uroepithelium damaged by the M. tuberculosis and, multiplying strongly, can create microbial films with insensitivity to antibacterial drugs.
