Abstract

Background. Ukraine has undergone significant medical, demographic and social changes, which are characterized by aging and significant shifts in the structure of morbidity and mortality of the population in recent decades. It requires the creation and development of an affordable, high-quality and effective the Palliative and Hospice Care (PHC) System in Ukraine should be an integral component of the Health and Social Care System. PHC System is a modern patient-family-oriented humanitarian approach that can most adequately address medical, psychological, social and spiritual needs and ensure an adequate quality of life for palliative patients (PP) and their families, contributes to the PP human dignity maintenance at the end of life care. The purpose of the study is the analysis and synthesis of modern approaches and forms of the PHC provision to the population and analysis of the implementation of the "Program of Palliative Care in the Outpatient Setting in the Dnipro City for 2018-2021". Materials and methods. The data of medical statistics, national and international regulatory documents and literary sources, methods of system and structural-functional analysis, bibliosemantic and statistical research methods were used in the study. Results. An analysis of the medical and demographic situation and the state of the PHC provision in Ukraine and in the Dnipro City showed some regional features, in particular, the small number of inpatient facilities/beds for PP and the lack of multidisciplinary PHC visiting teams subordinate to the Dnipro City Council Health Care Department; the significant need to provide PHC to patients at home and the need to develop and provide a collaborative mechanism for primary health care physicians, hospitals and social care institutions; great interest and assistance of the Dnipro City Council and the Health Care Department in the modern affordable, effective and safe PHC services development and implementation to the Dnipro City population. Conclusions. The Dnipro City Council support to create modern affordable, effective and safe PHC service is an important condition for adequately providing the Dnipro City population needs in PHC. It was reflected in the "Program of Palliative Care in the Outpatient Setting in the Dnipro City for 2018-2021" approved by the Dnipro City Council, that should ensure effective interdepartmental coordination and cooperation between health care facilities and social care institutions, continuity and PHC multidisciplinary and interagency approaches providing through the multidisciplinary mobile teams to provide PHC at home in each district of the Dnieper City creation and development, to ensure coordination and continuity of inpatient PHC in hospitals. The inpatient PHC provision in the Dnipro City is insufficient and makes up only 40.0% of the need. 72,1% of incurable patients who received PHC in the Dnipro City for 2018 are PP with malignant neoplasm; most of the PP was in the older age category - PP over 75 years old accounted for 35,4%. It corresponds to the average indicators for Ukraine. 944 (78.4%) PP received adequate analgesics therapy, of which, thanks to funding, provided for by the “Program for the provision of palliative care on an outpatient basis in Dnepr for 2018-2021” 703 (58.4%) PP - for preferential recipes for a total of UAH 1,628,786, or in terms of one patient – 2,316.9 UAH; 202 PP who received PHC on an outpatient basis were provided with care products for a total of UAH 830,217, which is UAH 4,110. for everyone. The introduction of PHC international standards and experience significantly improve of the incurable patients needs provision, and improve the quality of life of PP and their families. It determines the great medical, social, humanitarian and economic importance of PHC as innovative direction of the health and social care system.
