Abstract

Клиническое руководство является результатом согласованного решения экспертов, принятого на основании тщательного анализа данных литературы, а также рекомендаций зарубежных руководств − Global Initiative for Chronic Obstructive Lung Disease (GOLD) пересмотра 2020 г. и Clinical Guideline No 101. Chronic obstructive pulmonary disease. Management of chronic obstructive pulmonary disease in primary and secondary care (NICE). В документе представлено обновленное определение ХОЗЛ, даны новые подходы к классификации и диагностике заболевания, предложены современные схемы лечения, основанные на результатах многоцентровых международных клинических исследований, что обосновывает высокую доказательность такой терапевтической тактики. Изложены новые принципы распределения больных на клинические группы, основанные исключительно на оценке субъективных проявлений ХОЗЛ, представлены алгоритмы начальной и поддерживающей терапии для каждой клинической группы больных, строго определены показания для назначения ингаляционных кортикостероидов, основанные на учете количества эозинофилов в периферической крови, новые подходы к лечению обострений заболевания. Руководство адресовано врачам всех звеньев оказания медицинской помощи − семейным врачам, терапевтам, пульмонологам, аллергологам, кардиологам, реабилитологам. Clinical guideline is a result of coordinated decision of experts, based on thorough literature data analysis and data from foreign guidelines – Global Initiative for Chronic Obstructive Lung Disease (GOLD) revisions of 2020 year and Clinical Guideline № 101. Chronic obstructive pulmonary disease. Management of chronic obstructive pulmonary disease in primary and secondary care (NICE). Renewed definition of COPD, the approaches to classification and diagnosing, contemporary treatment options, which rely on multicenter international clinical trials data, suggesting high clinical evidence quality, are presented in this document. New principles of distribution of the patients into clinical groups, based solely on evaluation of clinical characteristics of COPD, algorithms of initial and maintenance therapy for each clinical group of patients are presented. Indications for the use of inhaled corticosteroids are strictly defined, based on peripheral blood eosinophils count. New recommendations for the management of exacerbations are described. Current guideline is intended for use by all physicians — family practitioners, internal diseases physicians, pulmonologists, allergologists, cardiologist and rehabilitation specialists.
