Abstract

Мета роботи. Підвищення ефективності лікування хворих на акантолітичну (вульгарну) пухирчатку глюкокортикостероїдами за даними вивчення рівня NO-синтази на різних етапах терапії. Матеріали та методи. Під нашим спостереженням перебувало 15 хворих на акантолітичну пухирчатку (АП) у віці 58-74 роки (чоловіків – 10, жінок – 5). Матеріал (кров) вивчали тричі: до і після терапії, у період амбулаторного лікування підтримуючими дозами гормонів. У 9 з 15 хворих до початку терапії встановлено артеріальну гіпертензію, а 8 хворих до вступу до стаціонару амбулаторно самостійно приймали глюкокортикостероїди. Рівень NO-синтази визначали за методом Гріса. Концентрацію NO розраховували за калібровочною кривою з перерахунком на загальний білок і представляли у мкмоль/г білка/хв. Результати та їх обговорення. У всіх досліджених хворих, на відміну від здорових осіб, до початку терапії був статистично високий (р < 0,05) рівень NO-синтази. Більш високий рівень NO-синтази до лікування виявлено у хворих на АП, які попередньо не отримували глюкокортикостероїдів, а також при супутній артеріальній гіпертензії. Після закінчення стаціонарної терапії рівень NO-синтази знижався майже у 2 рази (р < 0,05), а в період амбулаторного лікування підтримуючими дозами глюкокортикостероїдів продовжувався знижатись (р < 0,05). Висновки. У хворих на АП виявлені суттєві зрушення рівня NO-синтази як у бік підвищення, так і зниження, що потребує терапевтичної корекції. Тому ми рекомендуємо вивчення рівня NO-синтази у хворих на АП на різних етапах лікування глюкокортикостероїдами як прогностичний фактор для вибору адекватних доз глюкокортикостероїдів (для зниження рівня NO-синтази), а також статинів (для зростання рівня оксиду азоту).Objective. Improving the efficiency of treatment of patients with acantholytic (vulgar) pemphigus by the use of corticosteroids according to the data of the NO-synthase level at different stages of the therapy. Materials and methods. We examined 15 patients with acantholytic pemphigus aged 58-74 years (10 men and 5 women). The material (blood) was studied three times: before and after the therapy and during the period of outpatient treatment by maintaining doses of hormones. Arterial hypertension was revealed before the therapy in 9 out of 15 patients. 8 patients used corticosteroids by themselves before admission to hospital. The level of NO-synthase was determined by Gris method. The NO concentration was calculated from a calibration curve based on the total protein and expressed in µmol/g of protein/min. Results and their discussion. All the patients having been studied, unlike healthy individuals, before the therapy had statistically high (p < 0.05) levels of NO-synthase. The higher level of NO-synthase before treatment was established in patients with acantholytic pemphigus, who previously had not received hormones, as well as in ones with concomitant arterial hypertension. After inpatient therapy, the level of NO-synthase decreased almost two times, and during outpatient treatment by maintaining doses of corticosteroids the level continued to decrease (p < 0.05). Conclusions. Patients with acantholytic pemphigus revealed significant changes in the level of NOsynthase in the direction of increase as well as of decrease, that required therapeutic correction. Therefore, we recommend to study the NO-synthase level in patients with acantholytic pemphigus at different stages of their treatment by corticosteroids as a prognostic factor for the selection of adequate doses of corticosteroids (to reduce the NO-synthase level), and statins (to increase the NO-synthase level).
