Abstract

В останні роки досить часто обговорюється питання розвитку псоріазу на тлі різних коморбідних станів і, зокрема, гіпертонічної хвороби. Розглядаються механізми формування, епідеміологічні особливості, клінічні аспекти поєднання нозологій. З метою вивчення особливостей формування псоріазу на тлі гіпертонічної хвороби у чоловіків, обстежили 53 хворих, які страждають як на псоріаз виключно, так і на псоріаз у поєднанні з гіпертонічною хворобою. Встановлено, що існують певні відмінності у клінічних проявах і перебігу псоріазу на тлі нормальних і підвищених значень артеріального тиску. Чоловіки з коморбідною патологією у середньому на 5 років старші за чоловіків, які страждають лише на псоріаз. Середній вік пацієнтів, які страждають на псоріаз із гіпертонічною хворобою, склав 53,3 ± 4,7 року, а з нормальними значеннями артеріального тиску – 48,3 ± 3,9 року. Є також відмінності щодо індексу маси тіла – у цілому превалює кількість пацієнтів з нормальною або підвищеною масою тіла у межах ожиріння 1 ступеня. У групі пацієнтів з коморбідністю майже 2/3 пацієнтів страждають на тяжкі форми дерматозу – артропатичну, дифузійну та ексудативну. Ці дані підкріплюються результатами підрахунку індексу PASI, який у групі коморбідної патології зареєстровано на рівні 32,9 ± 2,8, а у групі з нормальним значенням артеріального тиску – 27,4 ± 2,5. Пацієнти із зазначеними тяжкими формами мали також порушення добового ритму артеріального тиску у вигляді недостатнього його зниження у вечірні та нічні години.In recent years, the issue of psoriasis development against various comorbid conditions, and in particular, of essential hypertension had been discussed quite often. The formation mechanisms, epidemiological features, clinical aspects of the nosologies combination had been considered. To study the features of the formation of psoriasis against a background of arterial hypertension in men, 53 patients with isolated psoriasis as well as one combined with essential hypertension were examined. It was found that there were certain differences in the clinical manifestations and course of psoriasis against a background of normal and increased blood pressure. Men with comorbid disorders were, on average, 5 years older than men suffering from psoriasis only. The average age of patients with psoriasis and hypertension, was 53.3 ± 4.7 years and ones with normal numbers of blood pressure – 48.3 ± 3.9 years. There were also some differences with respect to the body mass index – in general, the number of patients with normal or increased body weight within the limits of the first-degree obesity was prevalent. In the group of patients with comorbidity, almost 2/3 of the patients suffered from severe dermatosis – arthropathic, diffuse and exudative ones. These findings were supported by the results of the counting PASI index, which in the group of comorbid disease was registered at the level of 32.9 ± 2.8, while in the group with normal blood pressure it was 27.4 ± 2.5. Patients with such severe forms also revealed the disturbances of circadian rhythm of blood pressure in the form of its inadequate decline during the evening and night hours.
